Travelers Army Medical Center was commissioned by Colonel General Robert S. Richardson Jr., the military governor of the Territory of Hawaii during World War II. General Richardson hired York and Sawyer, a New York architectural firm, to design a modernist medical center. Noted local landscape architect Robert O. Thompson designed the landscape in harmony with modernist architecture to create a "comfort station" for the emotional and physical recovery of American servicemen. At the outbreak of World War II, the Visiting Army Medical Center had a capacity of 450 beds and was later expanded to 1,000 beds by the addition of barracks-style buildings. Plans for the new Army Medical Center on Moanalua Ridge were drawn up in 1942 and construction was completed in 1948. It was officially dedicated on September 10, 1948.

According to a 1944 contract document, Tripler Army Medical Center's signature coral pink color was a personal choice of General Richardson, who borrowed the color and many other design elements from the Royal Hawaiian Hotel in Wake. The Royal Palace was one of the entertainment venues for American military personnel during World War II and was also designed by landscape architect Robert O. It was Thompson's design.

According to Tripler spokeswoman Margaret Tippy, another reason for the pink paint is that retired Col. Robert B. Tripler's chief architectural secretary.
